Obtain from school in person or download from school website.
Please complete the form and return it along with your child’s photo(passport size), the original and photocopy of your child’s birth certificate and four envelopes with the child’s name, the address and a stamp.
When interviewing NCS children, interpretation and/or translation service for applicants
will be provided where necessary; or parents and children are allowed to be accompanied
by a Chinese speaking relative/friend to facilitate communication.
